New Client Forms

All new Clients need to fill out a new client intake forms. These forms help us design a better program for you by providing us information on your current and past health status, goals, habits and previous experience. If you are coming in for an initial assessment please complete these forms and bring it with you.

Three Day Food Diary

For Clients who wish to include nutrition guidance in their package you will need to fill out a 3 day food diary. This helps us understand how we can help you and is necessary prior to beginning the nutrition program. Try to pick 3 days in a row that are “average” days for you as this will give us the clearest picture of your current diet. Also, be precise, include measurements and all foods. For example if you write down spaghetti, include what was in the sauce (kind of meat, any vegetables, cheeses etc).
